Dear Doctors of Chiropractic,I want to personally welcome you and thank you for partaking in this important and beautiful mission. These are the steps that you need to follow to help me process you as part of the mission: Complete and fax back the registration form. This will allow me to make hotel reservations in Panama. (link on the left) I need a copy of your chiropractic license and chiropractic diploma. Please mail them to: Lina Sigafoosita Ocon 123 E. Alisal St. Salinas, CA 93901 If you make your own travel arrangements make sure you send me a copy so we can pick you up at the airport. Registration fee is $149.00 per DC and $50.00 for staff, spouses or students. I will charge the registration fee to your Visa, MasterCard or Discover card on January 15th of 2002.
